We provide outpatient and intensive outpatient services for individuals struggling with substance use disorders (SUD).
According to SAMHSA; Substance use disorders occur when the recurrent use of alcohol and/or drugs causes clinically significant impairment, including health problems, disability, and failure to meet major responsibilities at work, school, or home.
If any of that sounds like you or someone you know, we are here to help. We will set you up with your first assessment appointment and help determine what treatment course is best for you. Listed below are the services we provide to help in the recovery process.
Call our helpful reception (208) 321-0160 and the receptionist will guide you through a few simple questions to get you started. These questions will include basic information about the person needing services, what services are being sought, insurance (if being utilized), scheduling preferences, and any other information that you feel would be pertinent to your treatment.
If sending a referral, please fax referral to (208) 321-0221. Please make sure to include in the referral all of the above information so that our dedicated team can reach out to the person seeking services as soon as possible.
Once the receptionist gathers all the relevant information to get the person started in the SUD program, the receptionist will send the intake paperwork to you via email or, if requested, via postal mail. This packet of paperwork needs to be completed before you come in for your first appointment. If the paperwork is not complete prior to your appointment, please make sure to come to your first appointment 30 minutes early to complete before your intake assessment appointment.
Please make sure to have all required information and paperwork with you if this treatment program is being court ordered. We will need a release of information for all those that you want involved in your treatment.
Once you have all required paperwork filled out, you will be seen for an assessment appointment. This might be a Comprehensive Diagnostic Assessment, ASAM, GAIN, or DUI Eval. These appointments can last anywhere from 1 hour to 2 hours. Please be prepared to be present in person the whole time.
Once your appointment is complete, you will check out with the reception and get your following appointments scheduled, as suggested by your assessor.
You're all set! You will be contacted by our SUD Admin if there are any other questions, if not, you will continue attending your scheduled appointments as agreed upon during your first assessment appointment.
